To take good care of the dog and not part with him/her to any other person or body except to the Trust.

Under no circumstances to allow the dog to be kept outside or live in an outside building.

Not to allow the dog to roam free or worry livestock, thereby causing a nuisance to others.

In the event that the dog goes missing to take all practicable steps to find him/her as soon as possible and to inform the Trust and relevant authorities immediately.

To allow a bitch to be spayed by the Trust, failure to allow this will result in the bitch being reclaimed by the Trust. If a bitch becomes pregnant before spaying, puppies are the property of the Trust.

As new custodian of the dog, to be responsible for all veterinary charges and treatment that the dog may require and indemnify the Trust against all liabilities.

To keep the dog fully vaccinated at all times by means of an annual booster and de-wormed; under the advice of a qualified Veterinary Surgeon.

Not to have the dog put to sleep without the Trust's permission, unless a qualified Veterinary Surgeon certifies that it is essential to save the dog pain and suffering.

To allow a representative of the Jerry Green Foundation Trust to visit the dog and check on it's welfare at any time. In the event that the Trust's representative decides that the conditions in which the dog is kept and/or his/her state of health and wealth are not satisfactory; to allow the dog to be reclaimed by the Trust.

To notify the Trust of any change of address where the dog is kept.

To make full provision should the dog be left unsupervised.
